If your insurance is not listed here please contact our office to see if your insurance is one we are contracted with.
If you do not have insurance we may be able to offer you a prompt pay discount that is available ONLY on the date the services are provided. If you are uninsured (no insurance) the immunizations may be covered under the Vaccine for Children’s program. All balances are due within 30 days from the date on your statement. If you need to make other arrangements please contact our Toni or Kristina at 541-772-5548. We offer monthly payment plans in effort to work with you in paying your balance due.
We assign all unpaid delinquent accounts to an outside collection agency that reports to a national credit bureau. The Outside Collection Agency may add additional penalties and interest to the unpaid amount.
The Kids Clinic acknowledges and appreciates the many complexities to a divorce or non-intact family. We understand your situation is stressful. The Kids Clinic will not be responsible for disputes between parents due to a divorce or non-intact family. The custodial parent or the parent who brings the child(ren) to the clinic are financially responsible for the account of the child(ren). If the courts hold a specific parent responsible for providing health care, the dispute is between the parents and will not be arbitrated by the Kids Clinic. The parent who is responsible for the account is responsible for payment of all services not covered by insurance.
Our practice may use and disclose your child’s Individual Identifiable Health Information (IIHI) in order to bill and collect payment for the services and items provided by us for your child. For example, we may disclose your child’s IIHI as follows:
- To contact your child’s health insurer to certify that your child is eligible for benefits (and for what range of benefits),
and we may provide your child’s insurer with details regarding your child’s treatment to determine if the insurer will cover, or pay for, your child’s treatment
- To obtain payment from other third parties that may be responsible for such costs.
- To bill you directly for services and items.
- To other health care providers and entities to assist in their billing and collection efforts.
